Home from Hospital Support Service Coordinator (Derby Royal and Chesterfield Royal Hospitals)

Salary:
£26845 (pro rata for part time posts)


Hours:35 hours per week


Contract Type:
Fixed Term until 31st March 2024 (with the possibility for an extension)


Travel expenses:
Public transport at cost or 45p/mile

Based:

Royal Derby and Chesterfield Royal Hospitals (Post holder will be expected to split time across both sites and travel between them).


In 2021 the service received an additional investment to pilot the work of two hospital-based coordinators whose role is to support the discharge process based within acute hospitals.

This has proven to be highly successful and as a result of the impact delivered we have received further funding to expand the team.

We are now looking to recruit a further 3 roles to the hospital-based Coordinator Team.

It is our hope that with a 5 strong team we will be able to further demonstrate the difference these roles make in supporting hospital discharge and secure further funding beyond March 24.


Purpose of job


To support the co-ordination of a person-centred Home from Hospital Support Service (HFHSS), respond to individuals within a specific geographical area.

To assess need and support planning for discharge from hospital, liaise with and support area coordinators to deliver and review a service response aimed at achieving identified outcomes for service users.

This role provides support to both Royal Derby and Chesterfield Royal Hospitals with a special emphasis on pre-admission planning.

This role will involve liaising with Pre-operative assessment teams and patients who are going in to hospital for pre-planned or elective surgery.

Community based support can be established in advance to then reduce the length of stay in hospital post-surgery.


Main duties

  • Actively promote the HFHSS and to develop working relationships and referral pathways with the Integrated Discharge Hubs at the acute hospitals and specific communitybased hospitals
  • To work closely with Hospital Discharge Coordinators on the wards to identify patients prior to discharge that may need HFH support.
  • To assess the patient's suitability for HFH support prior to discharge.
  • To assess and discuss with the patient their needs on leaving hospital and undertake and document a personcentred holistic assessment of need
  • To begin planning the support required prior to them leaving hospital with area HFH coordinators
  • To provide a 'Day after Discharge' call to all clients discharged ensuring that they are linked with the districtbased service
  • To oversee the implementation of a personcentred delivery plan in response to the assessed need (as above), linking with HFH District Coordinators and other communitybased services where appropriate
  • To support the handover to HFH District Coordinators upon discharge and assist where appropriate
  • To assist in reviewing service user progress
  • To identify any gaps/ needs that the service cannot assist with and record those for feedback to Hospital Discharge Teams.
  • To carry out appropriate risk assessments
  • To record all activities and keep up to date and accurate case notes on CRM
  • To produce service and monitoring reports
  • To work within agreed service standards and ensure contractual targets are met.
  • To comply with all organisational policies and procedures and be fully committed to safeguarding, confidentiality and equality & diversity when carrying out duties
  • To contribute to the overarching aims and work of the organisation as required
To support the above work the post holder will be expected to:

  • Work closely with the Hospital Discharge Teams and the HFH District Coordinators to ensure seamless hand overs and team working and;
  • Monitor their work, keep accurate records and produce reports and statistics as required;
  • Attend staff and team meetings, staff training and organisational development days and the Annual General Meeting;
  • Liaise with staff in partner agencies;
  • Be responsible for their own health and safety and the health and safety of others who may be affected by their acts or omissions at work;
  • Comply with SDCVS policies and procedures;
  • Carry out any other reasonable duties as agreed with their Line Manager.
