A virtual ward is a safe and efficient alternative to NHS bedded care that is enabled by technology and home visits.

This innovative approach enables us to deliver safe, high quality and personalised care to people at home where they would rather be.

We know that people recover faster and better in their own environments and our dedicated teams of trained staff endeavour to support this.

A hybrid model of virtual, telephone and face to face consultations can be delivered as required, allowing patients to recover in their own home with continued clinical support.

The Hub team currently provides support for Respiratory, Frailty, Heart Failure and Home IV patients, with the desire to develop and expand further pathways.

Our Nurse Associates provide holistic, person-centred care and support for service users of all ages in a variety of settings.

You will be bridging the gap between health care support and registered nurses, whilst working safely within cope of practice and competence at all times.

Key responsibilities include:

Undertaking holistic assessments

Implementing care programmes

Assessing and monitoring service users health needs

Base location:
John Milton Clinic, Henbury

Service hours: 8am-8pm, 7 days per week including bank holidays. Your shifts will be allocated on a rota basis and will incorporate a variety of shifts within the above hours
