An exciting opportunity has arisen for a motivated and skilled Mental Health Nurse, Occupational Therapist or Social Worker to join our top performing Early Intervention in Psychosis Team in Bolton as a Care Coordinator.
You will be joining a client centred, dynamic and friendly team, who prides themselves on delivering high quality, evidence based interventions for those experiencing first episode psychosis.
The Early Intervention in Psychosis Team (EIT) works with people years old) who are experiencing a first episode of psychosis.
It is a multidisciplinary team consisting of staff from a variety of disciplines, including Psychiatry, nursing, social work, occupational therapy, and psychology.
One of the key aims of the team is to reduce the duration of untreated psychosis.We achieve this through offering full care-coordination, evidence based group and individual psychological interventions (including cognitive behavioural therapy and Family Interventions), support with medication management, support with accessing employment, education and vocational activities and support for families and carers.
The role of the Care Coordinator is integral to supporting a person on their recovery journey, through the CPA process.
If you are a qualified Mental Health Nurse, Occupational Therapist or Social Worker and would like to join a Top Performing Team making a difference for people experiencing first episode psychosis, this is the job for you.
- To hold and manage a caseload of service users who have
psychosis, including needs assessments, care planning and
delivering psychosocial interventions.
- To complete specialist assessments for referred service
- To undertake risk assessment and risk management for
professionals on acute and chronic aspects of risk
assessment and risk management.
- To work as part of our integrated MDT, and to participate in
appropriate management plans for all service users who are
under the care of the team.
- To provide advice, guidance and consultation to peer and
by attendance at team referral and review meetings.
- To participate in the duty/assessment rota operated by the Early
reater Manchester Mental Health (GMMH) Foundation Trust employs over 6,400 members of staff, who deliver services from more than 160 locations.
