Description

To liaise with a wide variety of colleagues from time to time, but the ability to work effectively with childrens social care staff, as well as Trust colleagues, will be particularly important.

Maintain effective personal contact with patients, relatives, and carers, being sensitive to their needs.

Maintain up to date patient records electronic (iLCIP real time) and paper based ensuring that patient confidentiality is always maintained.

Ensure all practice and discharge procedures are evidence based, safe and do not put the patients at risk.

Ensure that all nursing procedures are carried out in accordance with Trust policies and procedures. Set, monitor, and maintain standards for discharge in conjunction with other members of the Multidisciplinary Team. Develop information packages for staff and patients on the discharge process.

To prioritise own workload to facilitate a seamless and safe discharge of patients back into their own environment or new one that meets their present needs.


To contribute to the local resolution, investigation or any informal or formal complaints and carry out follow up action for those that specifically relate to discharge.

Ensure early preparation of Take Home Medications in consultation with medical and pharmacy staff. To provide leadership to promote nursing care which encourages rehabilitation and patients independence leading to prompt discharge from hospital.
