Description
To develop and demonstrate a high degree of competency and expertise in the provision of Community Mental Health. To be expert in the provision of evidence-based care. To provide clinical leadership for the CMHT.

Demonstrate applied knowledge of mental health legislation, related codes of practice and national and local policy and guidance Ability to identify the presence of mental disorder, the severity of the disorder; and determine whether the disorder is of nature or degree warranting compulsory detention To advise and lead on clinical risk assessment and management and to ensure that risk assessments and management plans are updated regularly.


To lead on cases that are complex in nature and support practitioners in their role as key workers for individuals referred into the service.

Provide expert advice to clinical staff within the team and within other services on the assessment, planning, implementation and evaluation of care and treatment of service users.

Provide expert advice to clinical staff within the team on the assessment and management of the physical health needs of service users To develop and exercise high levels of judgement, discretion and decision making in client care and management.

To interface between nursing, allied health professionals and medical colleagues to ensure that service user needs are being met.

Advise on the appropriateness and suitability of referrals into the service providing feedback to the referrer where appropriate.

To ensure all areas of work of the post holder promotes service user and carer involvement and promotes high quality care and social inclusion.

To develop extended practice/new ways of working that fits and supports the clinical core competencies and role of Advanced Practitioner.


These Clinical Core Competencies will include:
To analyse and interpret history, presenting symptoms and physical findings to develop differential diagnoses and appropriate formulations.

  • To advise patients and staff regarding drug regimes, sideeffects and interactions and, as a nonmedical prescriber, prescribe and review medications based on efficacy, safety and financial expenditure, from the appropriate formulary To integrate psychosocial interventions and nondrug treatments into specialist clinical management plans To carry out physical history and physical examination using precision diagnostic instruments which are within the post holders sphere of competence To order, perform (as appropriate), and interpret screening and diagnostic tests To evaluate results of interventions using various outcome criteria, revise treatment plans accordingly, and consult / refer when appropriate To provide consultancy within the defined clinical core competencies To identify, review and develop local service protocols/policies/guidelines with Managers and teams, in support of new ways of working and ongoing service development Data, Audit and Evaluation. To develop the dissemination of best practice and support the implementation into clinical provision. To lead on regular audit and evaluation of care planning, assessment, and documentation standards within the team. To ensure all activity undertaken within the role is recorded accurately utilising the agreed electronic and manual recording systems within the team.


To contribute to achieving the agreed performance standards and targets for the team To initiate and co-ordinate a programme of research and audits that support the role and demonstrates effective outcomes of care and wider learning.

To participate in any required internal or external evaluation undertaken as a consequence of the programme. To maintain and provide data as requested by the Trust and/or SHA.
