Pennine Care NHS Foundation Trust is delighted to develop the new Community Rehabilitation Team to support service users experiencing complex psychosis, who are ready to step down from inpatient rehabilitation services into the community.
We aim to reduce the length of inpatient stay for service users in acute and rehabilitation wards, incorporating least restrictive practice, the NICE guidelines for complex psychosis and the Getting It Right First Time (GIRFT) national program.
We are looking to recruit a motivated and enthusiastic Social Inclusion Worker to join our supportive multidisciplinary team and contribute to this exciting endeavour.
The team has a strong emphasis on the Recovery Model, working with service users to maximise their independence and autonomy to improve their quality of life.
The team operates during office hours of Monday to Friday 9am-5pm.
Our social inclusion workers will support service users to access socially inclusive activities to maintain their wellbeing and to prevent relapse.

The role will involve working closely with a range of professionals to include our VCSE partners as well as inpatient services, community services and supported accommodation providers.

Essential Criteria:
- NVQ Level 4 or BTEC Higher National
- Experience of working with people with
- Experience of community working and/or
- Experience of working in rehabilitation
- Demonstrates ability to use initiative to
- Ability to work collaboratively with the
- Knowledge of the Recovery Model and
- Knowledge of rehabilitation principles
Desirable Criteria:
- Lived experience
- Experience working in low secure or
- Experience of providing supervision
- Experience of facilitating group work
- Experience of providing constructive feedback to peers
