Job summary
This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation.An exciting opportunity has arisen within the Watergate Community Nursing Team for a 30 hour Band 5 Community Nurse on a permanent contract.
The applicant will need to demonstrate excellent communication skills,
sounds ability to prioritise and time manage their own patient visit list within the Watergate Community Nurse Team.
Responsibilities
- Responsible for the assessment,
planning implementation and evaluation of care without direct supervision
to patients carers and relatives within their home environment enabling them to enjoy maximum independence
and optimum quality of life. - To work in partnership with patients
& carers to offer support advice promote informed choice.
