    About the Role

    A vacancy has arisen for a Social Worker on a fixed-term basis until March 2026. You will join our Continuing Health Care team, working across various bases in Nottinghamshire.

    Key Responsibilities

    • Carry out collaborative work with Health colleagues to improve outcomes through decision support tool meetings and joint funded reviews.
    • Ensure proportionate care and support is in place for individuals in need.
    • Work as part of a diverse team, comprising Team Manager, Service Advisors, Community Care Officers, Social Workers, and an Occupational Therapist.

    Requirements

    • A recognised social work qualification.
    • Registration with Social Work England.

    Working Arrangements

    You will work a 37-hour week, with flexible working arrangements in place, including job share schemes and hybrid working between office and home-based environments.
