An exciting opportunity has arisen for a 6 month secondment, to join the Elective Care Team as a full-time Transformation & Commissioning Partner.


The postholder will support the Head of Transformation & Commissioning with delivery of a range of identified transformation programmes and commissioning duties.

The Elective Care Transformation & Commissioning team is made up of a Director with a Head of Transformation & Commissioning, Transformation and Commissioning Partners, Project Support Officers, Ophthalmology Advisor (Clinical) and admin support.

The team work on a number of different large-scale projects across the ICS, alongside day-to-day commissioning and work closely with other departments across NHS Shropshire, Telford and Wrekin, such as Contracting, Finance, Quality, Primary Care and Medicines Management.


The postholder will work as part of the Elective Care Transformation & Commissioning team, utilising the standard approach to Programme Management, to deliver agreed key outcomes and objectives across a range of transformational programmes and projects during the 6 month secondment period.


The role includes programme management of a defined remit of projects or programmes of work, adhering to standard project management methodologies and due diligence, as well as general commissioning support on a number of specific areas or services.


NHS Shropshire Telford and Wrekin is responsible for planning and buying a wide range of health and care services for the whole of Shropshire, Telford and Wrekin.

These include GP and primary care services, hospital care, community healthcare and mental health services.

We also have a duty to monitor these services to ensure they provide a high level of care and are value for money.

We are part of Shropshire Telford and Wrekin Integrated Care System.

This means we work closely with our partner organisations across the county, such as the two local authorities, the 51 GP practices, the hospitals, and the voluntary and community sector, to plan and deliver joined up health and care services.


Working closely together, in a more joined-up way, means we can have closer links to our patients so we can develop more personalised local services which will improve the lives of our residents and reduce inequalities.


NHS Shropshire, Telford and Wrekin currently have an agile working policy, with a combination of working from home and occasional office based work or meetings.

Statement of Job Purpose

  • To ensure full clinical engagement is used to maximise commissioning and service redesign opportunities regardless of organisational boundaries.
  • To actively support the Head of Transformation and commissioning and other colleagues to deliver NHS STW's strategy and operational plans.
  • To support the development of NHS STW and widersystem operational delivery plans for the relevant area of responsibility, developing an annual delivery plan and take responsibility for delivering the changes and targets within it.
  • To lead the implementation of a programme approach to service redesign and commissioning in order to deliver high quality services in all care settings, ensuring that commissioning is consistent with national policy, local need and strategic priorities.
  • To be responsible for leading and managing a range of specific projects/initiatives within NHS STW and partner organisations, ensuring effective and timely implementation of programme outcomes and service benefits.
  • To take responsibility for translating programme and commissioning changes into business cases for review and approval by NHS STW.
  • To work with colleagues in the Contracting Team to ensure approved commissioning and redesign changes are incorporated into contracts, in particular, leading on health outcomes.
  • In partnership with the Head of Transformation and commissioning, to provide support to realise the transformational and commissioning change necessary across the health and social care economy to ensure sustainability.

Responsibilities:


  • To be the lead commissioning and transformation manager for specific service areas and respond to necessary internal and external requests.
  • To work with colleagues across NHS STW to ensure that there is effective collaborative working on strategy development and service redesign with key partners, including patient & public representative groups, the local authority, service providers and voluntary sector organisations.
