Job summary
An exciting opportunity has arisen for a highly motivated individual to join our friendly and proactive administration team. This is a full time post - 37.5hrs a week.


Our core opening hours are between 8.00 and 18.30 with two late evening opening until 20.00 on a Tuesday and Thursday.


Main duties of the job


The Clinical Services Care Co-ordinator is primarily responsible for supporting the Clinical Services Team with regards to the delivery of services offered at the practice and the wider Primary Care Network (PCN) across Canterbury.

The focus of the team is to ensure high quality care for patients through well-co-ordinated administrative systems, offering a holistic approach to improve patient outcomes and experience.


The role includes:


Providing administrative support to help deliver GMS and Non GMS work including the Quality Outcome Framework (QOF), Enhanced Services, AQP Contracts, PCN DES and IIF activities and any other schemes as required.

Liaising with and supporting the Practice and PCN Teams as well as other third party contractors.

Helping to improve systems and processes including utilising software/technology to improve patient care and outcomes.


Overview of our organisation


University Medical Centre is a busy GP Practice in Canterbury that provides medical services predominantly to the student population and also to the residents in our local area.

We serve a population of around 16,000 patients.

We are a 6 partner practice and have a variety of services that meets the needs of our unusual population.

We are a friendly practice that promotes team working, development and innovation.


Job summary:


Effective communication skills and teamwork are crucial in a multidisciplinary environment, helping ensure the quality and consistency of the services the practice and the PCN provides.

The post requires a pleasant and approachable manner with the ability to exercise tact and discretion as well as being able to use initiative.


The Practice will support continuing education and professional development in the form of attendance at study days, courses and with in-house training, as deemed appropriate for the benefit of both the individual and the Practice.


Key Responsibilities


Provide administrative support to the Clinical Services Team with regard to GMS and Non-GMS work including the Quality Outcome Framework (QOF), Enhanced Services, AQP Contracts, PCN DES and IIF activities and any other schemes as required.

Liaising with and supporting the Practice and PCN Teams as well as other third party contractors.


Support patients to manage their needs through answering queries, making and managing appointments and ensuring patients have good quality written or verbal information to help them make choices about their care.

Effective use of IT/software to assist in management of practice population including EMIS, Ardens and AccuRx etc


Utilise EMIS searches and reports, undertake auditing as directed including long-term condition and prescribing monitoring as well as wider areas such as population health for both practice and PCN.


Assist with mail merges and text messaging campaigns to support long-term condition management, vaccinations campaigns as well as other information which may include health promotion or patient feedback.

Support collection of data for Clinical Services Meetings and assurance reporting.


To attend practice, PCN and other external meetings as appropriate, supporting wider collaboration and co-ordination of systems and patient care.

Attend appropriate Quality Improvement meetings, moving forward any actions to meet deadlines.


To keep up to date with mandatory and role specific training as well as attend training courses and other events relating to the role.

Helping to solve day-to-day issues with IT, systems, workloads, etc as they arise

To keep up-to-date with the Policies and Procedures of the organisation

Undertake any other duties appropriate to the role.


Provide administrative support to improve patient outcomes and long-term condition monitoring including achieving the Quality Outcome Framework (QOF) indicators, supporting GMS contract requirements, PCN DES and IIF activity and other health promotion campaigns.

This will involve contacting patients, arranging reviews, liaising with the wider team and referring for onward management where appropriate.


Provide administrative support and help with coordination of appointments and home visits for severely frail patients (and others as required) including developing a single personalised care and support plan and co-ordination of MDTs within PCN.

Provide administrative support for in-house and external services including liaising with clinicians, patients and others, booking of appointments and co-ordinating care

Assist with medical reports, processing requests in a timely manner, liaising with the clinical team, third parties and ensuring GDPR requirements are adhered
