Pension:
Up to 9% (non-contractual)


Salary:
£25,474 per annum pro rata


Contract:
Fixed term until 31/03/2026


Hours required:17.5 Hours per week.


Location:
Yeovil with 40% of time spent in Bridgwater


Holiday:23 days plus bank holidays (FTE)


Job Context:

We are creating a new Community Rehabilitation Team, which will specialise in working with people with longer-term mental health support needs.

The service will offer specialist rehabilitation assessment, support and assistance to people with more complex mental health problems.

The service's focus will be on supporting people to develop skills and confidence to live independently and to work towards each person's own recovery goals and improved mental well-being.

The service has high aspirations of recovery and well-being for everyone it supports.


Job Purpose:
Our aim is to reach all of Somerset's communities and to ensure we do this we have developed strong links with a range of organisations across the county


Support will include:

  • Tools for wellbeing and recovery.
  • Psychological support.
  • Emotional support.
  • Help to find appropriate housing.
  • Links to community resources and activities.
  • Practical support.
  • Help to access education and training.
  • Support to find suitable volunteering and employment.
  • One to one support and group activities.


Support will be offered 7 days a week, 10am - 8pm & you will work on an agreed rota which will include working at the following venues: Somerset on site in Yeovil (The Foyer).

Team office space will be established in Bridgwater, and the role will include some home and community working.


Main Tasks:


  • Work as part of a multidisciplinary team (MDT) to coproduce care plans, crisis plans and other selfmanagement tools with the people accessing the service, including input from their carers where appropriate.
  • Work in a Psychological and Trauma Informed way to provide inclusive, responsive and holistic support tailored to each individual's needs, including agile/remote working where it meets needs and preferences.
  • Ensure service users are supported to make informed decisions about their mental wellbeing, maximising their independence and providing advocacy role where appropriate.
  • Independently identify and manage risk to ensure this is minimised. Identify when risks need to be escalated to senior members of the team.
  • Work in a safe manner using a positive risktaking approach, in line with the services health and safety policies.
  • Provide flexible support, including at inpatient and community locations, and support warm transfers to communitybased services.
  • Support people on an individual basis to settle into the home of their choosing and be part of their local community, accessing services that they are entitled to which benefit and interest them.
  • Build knowledge of available services and proactively share this knowledge with the wider team to the benefit of the whole client group.
  • Support individuals to explore and access hobbies and interests, as part of developing wellbeing and meaningful connections.
  • Support individuals, where appropriate, to have a clear focus on training and access to paid work, maintaining hope and optimism for the individual with high expectations for recovery.
  • We are looking for people who have experience of working with people with longterm mental health and complex needs including complex trauma, Learning Difficulties, neurodiverse needs and SEND.
  • You will have a strong commitment to ensuring the service ensures equality of access and support to those who may have experienced greater challenges to accessing services in the past.
  • Work with a range of needs including clients with protected characteristics, promoting equality and fair access to community resources and opportunities, challenging stigma and discrimination and advocating on individual's behalf where appropriate.
  • You will be confident in working autonomously and have excellent communication and teamwork skills.
  • Manage caseload, documentation and time effectively, ensuring up to date and clear record keeping in line with policies, making best use of supervision, training and staff development.
  • Collaborate with other team members to maximise service performance, meet targets and Service Level Agreements and promote effective communication and teamwork
  • Work within a rota system including evenings and weekends as required.
