Are you passionate about providing high quality patient care? The Liskeard-Looe Community Nursing Team are looking for motivated practitioners to join us.
This is an exciting and fulfilling role and you will need to be able to demonstrate a real enthusiasm for nursing people in the community especially those approaching the end of their lives as well as a desire for on-going professional development.
You should also have an understanding of the changing climate of care delivery in the community with the ability to travel independently across the area to fulfil the requirements being essential.

They will monitor the condition and health needs of people within their care on a continual basis in partnership with the multi professional team.
This role will support the Community Nursing team with organising the patients on the end-of-life or Continuing Care at Home caseload.
