Community Mental Health Practitioner - Bury, United Kingdom - Pennine Care NHS Foundation Trust
Description
The post holder will be expected to demonstrate high level of commitment, motivation and knowledge of working with people with severe and enduring mental health problems.
The role will require clinically effective assessment, care management skills and interventions in accordance with the Care Programme Approach and Care Act framework.
The post holders duties will include engagement with clients in the development of person centred, recovery focused care plans, supporting our experienced duty team with short-term pieces of work and supporting people managing crisis.
You will be dedicated to delivery, both as part of a team and working independently, of a high-quality, innovative, recovery focused and person centred care which meets the needs of people who use our service and their carers.
