Walking With The Wounded (WWTW) supports those who have served in the Armed Forces and their families, empowering them to regain their independence, thrive and contribute in our communities.
By providing an integrated Care Coordination service, WWTW will ensure these individuals are provided the breadth of care they need, as well as providing support to their families.
The Care Coordinator will be an impactful and crucial service to a small cohort, which is highly in need.
WWTWs Care Coordination service involves the assessment of needs for your client, the sourcing of appropriate support and the ongoing navigation of that support.
You remain the central point of contact as you support your service user to address their challenges; which may include a lack of secure housing, debt, social isolation, relationship breakdowns, lack of education and training, welfare issues or substance misuse.
The Care Coordinators will have a caseload of complex ex-servicemen and women. Working with local delivery partners, you will make referrals for specialist support, whilst retaining the overall caseworker responsibilities. Your role will be crucial to the success of the service users outcome.If you are an experienced caseworker with an understanding of working with more complex clients, we would like to hear from you.
