We are recruiting for a Housing Officer for the Aylesbury area, responsible for facilitating smooth transitions for patients from hospitalisation to permanent residency.

This role presents an opportunity to make a meaningful impact by collaborating with various external stakeholders.

Key Responsibilities:

Identify patients ready for discharge but facing housing and benefit-related challenges.
Assess patient housing needs and potential homelessness risks, crafting tailored discharge plans.
Liaise with local housing authorities and associations to access relevant housing services.
Share pertinent information with social workers and housing/homelessness services.

Qualifications and Requirements:

Proficient understanding of the benefits system, staying updated with its nuances.
Previous experience working with individuals grappling with homelessness and housing issues.
Familiarity with the discharge process for hospital patients.
Proven track record of supporting individuals with mental health disorders.
