Description
The successful post holder will work under the direction of a clinical lead, GP/Qualified Nurse and assist in the implementation of patient care both direct and non-direct.
Care coordinators play an important role within a Primary Care Network to proactively identify and work with people, including the frail/elderly and those with long-term conditions, to provide coordination and navigation of care and support across health and care services.
The successful post holder will be based at both Park Parade Surgery and 49 Marine Avenue Surgery in Whitley Bay, splitting the week between both sites.
