Description
To use advanced nursing skills and knowledge to provide autonomous holistic assessment, diagnosis and management of homeless patients presenting with acute undifferentiated and undiagnosed conditions and injuries, complex health needs and/or long term conditions To provide complete episodes of care based on the best possible evidence and work within clinical guidelines, utilising expert judgement to work outside these frameworks when necessary To provide advanced nursing care to people experiencing homelessness both in a clinic setting and during outreach to the street and other locations To work in partnership with other community teams / voluntary organisations in order to ensure the seamless delivery of care to the homeless population and proactive engagement with underserved groups To act as an advocate for people experiencing homelessness, assisting patients to overcome barriers to healthcare and supporting their own choices in regards to their care whilst promoting independence and self-care.
