We have an exciting permanent opportunity for a full time band 5 Care Coordinator in our Cambridge Team based in Cambourne.

CPFT intermediate care provides a discharge to assess model of care to move people out of hospital quickly.

Currently, Acute Hospitals with Cambridgeshire and Peterborough are being prevented from performing their vital functions because their beds are filled by almost well people who are not able to return to their own homes.

The discharge to assess model facilitates enables people to be home as soon as possible.

The post holder will provide leadership and management to the assistant care coordination staff and integrated care workers.

Working under the Senior Care Coordinator, the post holder will ensure the smooth running of Care coordination regarding patient flow, care planning, pending list management, referral management and quality standards.
