The Role

You will join a dedicated team of

As a
Support Practitioner, you will play a crucial role in supporting these individuals with all aspects of daily living, including cooking, personal care, meal preparation, administering medication and the management of personal finances.

You will support them to do the things they want to do out in the community and accompany them on any outings, including when attending health appointments.

You will take an active role in the planning and delivery of person-centred care and be supportive and encourage the people we support to express their views and wishes.

Our aim is to enable the people we support to live a fulfilling and valued life, where they can achieve their own personal goals and aspirations.

To be a great
Support Practitioner, you will be:

  • caring with a positive, can-do attitude
- able to demonstrate exceptional communication skills, including active listening
- committed to improving the lives of the people we support
- able to build and maintain positive, respectful relationships with different people, including other agencies and health professionals involved in care
- competent in the use of technology to enhance the support you provide
