Part Time (16 hours/week) (Band 4)

SWAGGA Primary Care Network (PCN) is a dynamic collaboration between sixteen practices across South Liverpool. We have a vision to improve patient care, tackle inequalities and provide outstanding support for our member practices.

We plan to make a lasting impact upon the communities we serve and are developing ambitious plans for the future.

As a result, a new and exciting opportunity has arisen for a Pharmacy Team Care Co-ordinator to join our rapidly expanding team as part of the SWAGGA Primary Care Network.


Who we're looking for?


This is a new role for SWAGGA PCN so we are looking for someone who can work as part of a team and show initiative to help us achieve our goals.

You will need to be a strong communicator with good administration skills but most of all, we're looking for people with passion and compassion.


The role


As a crucial member of the team, you will work alongside practices to maximise the impact of the PCN Pharmacy Team.

You will design and implement systems and processes to support the Pharmacy Team deliver against local and national targets and to fulfil goals set by the PCN leadership team.


Specifically, duties will include:

  • Using the clinical system to identify target cohorts of patients for intervention based on local and national schemes and priorities
  • Managing timetables for team members and organising the staff rota for all members of the pharmacy team
  • Data collection and production of monthly KPI reports
  • Supporting Pharmacy Team in conducting audits
  • Providing administrative support for staffing functions such as annual leave, sickness, new staff induction and mandatory training
  • Liaising with practicebased care coordinators to help delivery against team targets
  • Organising and attending relevant meetings with interested parties
  • Supporting administrative aspects of pharmacy team meetings
  • Provide a single point of contact for practice queries
  • Ensuring pharmacy team documents are kept up to date and to assist with correct formatting of these documents
  • Striving for excellence in the holistic delivery of optimal and efficient service
  • Contacting patients to arrange appointments on behalf of the pharmacy team

Our combined patient population is approximately 100,000 and we work very closely with our community team and other local Healthcare providers.

This includes Salaried GPs, Advanced Clinical Practitioners, Clinical Pharmacists, Practice Nurses, Social Prescribers and First Contact Physios.


The post holder will also be required to travel from practice to practice and to other venues during fulfilment of their duties.

You will also be required to complete online Care Coordinator training which is currently provided through NHS England.


Person specification

Knowledge and Experience

  • Core level of Maths and English
  • NVQ level 3 or equivalent experience in a health and/or social care setting
  • Understanding of health inequalities
  • Understanding and awareness of Primary Care systems and processes
  • Understanding of wider healthcare delivery including roles of core MDT members and role of Primary Care
  • Experience of preparing plans and reporting progress
  • Experience of coordinating and liaising with multiple stakeholders or individuals to meet specified outcomes
  • Experience of analysing and interpreting information and presenting results in a clear and concise manner
  • Experience of collating and disseminating voluminous and sometimes complex information
  • Experience of organising recurrent events
  • Evidence of administrative skills and robust record keeping
  • Understanding/experience of using tools to create individualised plans
