This report should encompass a detailed account of the claimant's medical history, incorporating insights gathered about how their health condition(s) impact their daily life and mobility. Additionally, include information about their ongoing medication and treatment regimen.
- Within the report, provide a comprehensive rationale outlining the reasoning behind your conclusions drawn from the collected evidence. Make sure to outline potential timelines during which the health condition or impairment is likely to affect the claimant.
