Description
Arranging discharges for the patients. Discussion with patients and relatives about what support is needed in the discharge process. Discussion with multidisciplinary team members including social services, the discharge management team and district nurses. Discussion with ward physiotherapists and occupational therapist. Liasing with discharge lounge to arrange transport home.
