We are looking for a Mental Health support worker (mainly working with children)

  • A
    Level 2 diploma qualification in care or equivalent is desired but not mandatory_

_Main Tasks and Responsibilities:
_


  • To work in collaboration with other health care professionals both internally and externally in providing the highest standard of care possible to all the young people with mental health needs.
  • To implement and adhere to the care plans and individualised risk management plans for all young people in our care.
  • To support the young people with all aspects of personal care as outlined within their care plans. To complete tasks such as encouraging prominent levels of personal hygiene, meal preparation and wound care, respecting dignity, privacy, and rights as faras possible.
  • To support young people with first aid and wound care as required within individual risk management plans and policies.
  • To assist in creating a safe, clean, and secure environment to the highest standards of health and safety for all young people, relatives, and colleagues.
  • Enable young people to gain independence, confidence, and competence in the following areas: Social skills/relationships, Personal care and hygiene, Daily living skills, Using community resources and facilities, Social, leisure and work activities, Selforganisationand coping abilities and Personal safety. To achieve this through practical assistance, support, teaching, advice, role modelling, encouragement, and positive feedback.
  • To work in a variety of setting outside of the home, transporting young people to places such as education colleges/school, places of work, the local hub to receive therapy, to various activities as chosen by the young people and to the home area in orderfor the young people to maintain relationships with family and friends.
  • Once trained to provide and perform clinical duties necessary to meet the young people's health needs on a daily basis. Ensure that young people are receiving advice, care, and regular health checks to ensure their physical and mental wellbeing. Promotenutrition, relaxation, exercise, and healthy lifestyles.
  • To adhere to the activities programme to ensure the young people receive a variety of daily stimulation and development both inside and outside of the home.
  • To safely and competently, administer medication to the young people in accordance with their prescription and their policy. To adhere to all medication administration processes such as ordering, storing, and recording on the electronic MAR charts.
  • To conduct daily checks of the homes finances and report any discrepancies to the management team.
  • To complete clearly written, accurate, contemporaneous records of involvement with young people and their families. Ensure documentation is up to date, accurate and completed daily, adhering to the code of practice.
  • To participate in the handover process of the home and ensure all documentation is accurate and reflective of the daily activities.
  • To play an active part in the young person's key team, creating and updating care plans, work collaboratively with the homes multidisciplinary team ensuring all actions have been completed.
