    Job summary

    As a PCN and General Practice Clinical Pharmacist you will work with our current Clinical Pharmacy team to further develop and manage medicines optimisation services within our practice. You will work with our other 2 PCN practice's and have the opportunity to build up relationships with the primary care network team. You will work to manage repeat prescription authorisation and reauthorisation, acute prescription requests, management of transfer of care and systems for safer prescribing. You will have the opportunity to develop areas of clinical interest in a patient facing role, develop or improve your prescribing skills and improve the management of patients with long term conditions. Mentoring and support will be provided.

    Main duties of the job

    Please see the Job Description for full duties but you will will provide primary support our Clinicians, Pharmacy team and Prescription Clerke with regards to prescription and medication queries. You will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice.

    You will provide clinical leadership on medicines optimisation and quality improvement working to targets for the quality and outcomes framework and enhanced services.

    You will ensure that the practice integrates with community teams to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice. You will work with other Pharmacists across our Primary Care Network working on the PCN DES requirements being fully supported throughout.

    We are an innovative and caring, teaching and training practice aiming to provide high quality care for an inner city area which has a vibrant and markedly diverse population, facing many of the exciting challenges and interests associated with health issues related to poverty, migrant health, multi-morbidity and varying culture and ethnicity. The practice has approximately 12,100 patients and the practice team includes 6 partners, 4 part time salaried GPs, a strong nursing team with ANPs, NPs, PNs and HCAs, link worker, pharmacists and chaplains, a strong administrative team and IT support.

    The PCN Clinical Pharmacist will enhance our Pharmacy team and need to be committed to improving health and putting patients first. Key qualities will be the ability to build effective relationships with patients, GPs, nurses, community Pharmacists, hospital and practice staff, and to work effectively as part of a team.

    Job responsibilities

    The post holder will act within their professional boundaries, supporting and working alongside the clinical team including pharmacist and prescription clerk as part of a multi-disciplinary team in a patient-facing role. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.

    Person Specification

    Experience

    Essential

  • Minimum of 2 years post-- qualification experience.
  • In depth therapeutic and clinical knowledge and understanding of the principles of evidence--based healthcare.
  • An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
  • Excellent interpersonal, influencing and negotiating skills
  • Demonstrate the ability to communicate complex and sensitive information in an understandable form to patients
  • Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
  • Qualifications

    Essential

  • Mandatory registration with General Pharmaceutical Council (GPhC)
  • Masters degree in pharmacy (MPharm) (or equivalent)
  • Specialist knowledge acquired through post- graduate diploma level or equivalent training/experience,
  • Independent prescriber, ACP trained and/or CPPE qualified
  • Desirable

  • Membership of the Royal Pharmaceutical Society
  • A member of or working towards Faculty membership of the Royal Pharmaceutical Society
