Hours of work: 37.5 hours

Annual Leave: 30 days plus bank holidays

up to £25,833 per annum

We are looking for a Social Prescribing Link Worker to work with GP practices in the Maidstone PCN area (Bower Mount Medical Practice, Brewer Street Surgery, The Shepway Medical Centre and The Vine Medical Centre) to improve the wellbeing of their patients.

Taking a holistic approach, you will connect people to community groups and statutory services to provide them with practical and emotional support.

You will need a positive 'can-do' attitude, high levels of resilience, excellent communication skills and the ability to motivate, empower, and support people to achieve their goals.
