It you are looking for a new challenge with a job that is varied, makes a difference to peoples journey though hospital, and means meeting and working with people then this is the role for you.

The In-Reach team works across three acute hospitals: Salisbury, Great Western (Swindon) and the Royal United Hospitals in Bath.

The post holder would be based in one of these hospitals and the role involves supporting safe and timely discharges for all Wiltshire in-patients in that hospital.
