The Role:

  • Join our dyanmic Older Adults
-
Commissioning Team, within Adults' Health and Care, and support the development of Extra Care Housing and Support Services across Hampshire.
  • Our team supports and enables individuals to live long, healthy, happy lives, promoting independence, across the county. Delivery of this vision and strategy includes working with Independent Living Care Providers and our Care Homes Market.

We are looking to expand our team with two ambitious individuals that are eager to join our friendly and innovative team, supporting the development of the Care Homes Market and Housing Services across several key areas.
We have two roles available, focussing on two different areas. One is focussed around Extra Care Housing, housing support services and Disabled Facilities Grants. The other will work to the Care Home and Health Portfolio, leading on Care Homes (Residential and Nursing), with an initial focus on the development and implementation of a Care Home Framework.

  • With your understanding and experience of either care, support services commissioning and/or contract management, you'll focus either on the development of housing related support services, or the development of the Care Homes market. Both roles will involve a range of commissioning activities, enabling you to

plan, develop and procure new services, manage contracts, conduct service reviews and maintain excellent provider and stakeholder relationships.

  • Using your
-
excellent communication skills, you'll engage and build trust-based relationships with multiple stakeholders ensuring we're working with our partners in the most effective way.
  • Your previous
-
service design and development, quality and monitoring experience together with your understanding of social care associated financial and economic challenges, will support you to develop service requirements and implementation plans and work with providers to successfully implement changes for the benefit of our residents.
