Position:
Adults Social Worker


Location:
Manchester City Council


Pay Rate:
£30 per hour


Are you an experienced Social Worker with a passion for supporting individuals with learning disabilities as they transition into adulthood? Manchester City Council is seeking a dedicated individual to join our Transitions Service.


Key Responsibilities:
Utilize your expertise in learning disability roles to provide comprehensive support to young adults transitioning into adulthood
Work within the statutory framework of the Care Act 2014, conducting assessments of need to ensure a smooth transition for individuals
Collaborate closely with multidisciplinary teams to develop tailored support plans that meet the unique needs of each individual


Requirements:

Previous experience or knowledge of working in learning disability roles is highly desirable
Available to work through to the end of March 2024 and able to start immediately
Familiarity with the statutory framework of the Care Act 2014
