    The Hounslow Community Recovery Service provides rehabilitation and reablement for adults in Hounslow following an acute injury, illness or change in health or functional status aiming to improve functional independence and safety.

    The service is provided in partnership with London Borough of Hounslow (LBH) and includes the LBH Reablement Team.


    These are new posts created to support the health needs of patients on the rehabilitation and reablement caseloads, providing direct nursing assessment and interventions alongside supporting, advising and educating the wider team to enhance clinical practice within the service.

    In this role you will work with people to improve their independence in management of their health conditions and treatments, for example, medication management, pressure care, prevention of exacerbation of long term conditions.

    These are new posts and so we are seeking applicants who are excited for a new challenge and have a desire to explore the opportunities for a new role within an established intermediate care team.

    You will be supported to complete your independent prescribing training as required.

    A discharge to assess pathway to support early transition from hospital to home

    Close working arrangements with local Integrated Discharge Hub to support joint discharge planning

    Integrated health and social care reablement service

    High quality patient centred intermediate care in peoples own homes to support functional independence and safety


    Close working with rapid response, neurorehabilitation, falls and district nursing, ensuring joined up care planning and joint working where appropriate.

    Hounslow and Richmond Community Healthcare NHS Trust (HRCH) provide a wide range of healthcare services for the population of London Borough of Richmond upon Thames and the London Borough of Hounslow.


    The Clinical Services Directorates have a wide range of responsibilities to manage, including co-ordinating and delivering community primary care services on behalf of the Trust.

    A wide range of these healthcare services are delivered by staff in the community, who include medical staff, paediatric therapists, therapy assistants, health visitors, specialist nurses, nurses, nursery nurses, paediatric phlebotomists and healthcare support workers with the support of administrative staff working in localities with GPs, other health professionals, early years, education, and social care professionals.


    The digitalisation of health services is underway with a requirement to become paper-light by 2023 through improving the use of digital tools and adopting mobile and agile working practices and increasing the efficiency and effectiveness of clinical delivery.

    Registered Nurse Level 1 with current registration with the NMC
    Community Nursing Qualification/Specialist Practitioner
    Non medical prescribing (V300)

    Able to reflect and critically appraise own performance
    Ability to contribute to 7 day working shift pattern

    Understanding of rehabilitation approach to health and self-management
    Demonstrable post-graduate experience in a relevant area working with people in a relevant community setting
    Experience of case management and care co-ordination
    Experience of audit evaluation to improve healthcare

    Evidence of ability to maintain and monitor high standards of care
    Evidence of ability to communicate both verbally and in writing at all levels in a multidisciplinary forum and with patients and carers
    Ability to travel to multiple sites car driver or competent cyclist
