Description
Please refer to the attached job description and person specification for full details of responsibilities. To develop a close relationship with Primary Care practices, attending practice meetings and having regular informal contact.

To enable practitioners in Primary Care to develop their skills and confidence in assessment and treatment of mental health problems.

To liaise with key services within the neighbourhood who are supporting individuals with complex mental health problems outside of CPFT.

To offer outreach work to community groups where there is evident inequality of access. To triage referrals from primary care. To offer face to face assessments to more complex patients. To utilize the local H.A.Y resource to help individuals find support in their local community where appropriate.


To attend weekly clinical meetings with consultant and senior nurse colleagues working in the MDT to ensure that patients in the sector receive intensive and prompt treatment when needed.

To provide educational opportunities for Primary Care Team Workers.

Co-ordinate annual leave / bank holiday / study leave arrangements if needed by the Operational Manager ensuring staffing levels remain sufficient to meet service needs.
