    Job summary

    We are looking for an experienced Mental Health Practitioner who has a keen interest in working in a supportive multi-disciplinary setting with patients experiencing mental health difficulties. Historically, the role of the duty worker was shared between care-coordinators on a rota basis but we as part of the community mental health transformation we are hoping to establish this new role within the team as a sole, full-time, duty worker.

    This is an exciting opportunity to work within a community environment, without the expectation that you manage a caseload.

    The usual hours of work are Monday to Friday 9am - 5pm.

    Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.

    Main duties of the job

  • To maintain patient safety, in collaboration with patients, carers and families.
  • To maintain professional standards of care at all times.
  • To be able to recognise the potential for or signs of patient harm, abuse or neglect and taking all reasonable steps to protect the patient or anyone else who is at risk of harm.
  • Appropriate escalation of any concern of any form, to the multidisciplinary team and timely action of any outcomes from these MDT discussions.
  • Undertakes risk assessments.
  • Responsible for decision making and coordination in a crisis situation with the support of the wider multidisciplinary team.
  • This role does not involve management of a caseload however, the post holder is expected to be involved in the care of people with complex and varied mental health needs who may present to the team when their key worker is not available or is absent from work and an urgent response is required.
  • Working to support the team where more intense support is required, but crisis referral is not warranted.
  • To liaise with other services as required.
  • Being part of joint appointments with other members of the multi-disciplinary team where required due to a patient's risk profile.
  • Support members of the team with other specialist assessments, as required.

    Job responsibilities

    The main responsibility of this role is the confident and sound clinical decision-making ability, alongside appropriate use of the multidisciplinary team in order to ensure that any patient concern is handled appropriately and steps are taken to ensure that the correct support is implemented.

    The duty worker may be expected to complete urgent assessments and on occasion deliver planned interventions, however they will not be expected to manage a caseload of patients. It is expected that the duty worker would support the team with service users who are struggling with their mental health but where crisis intervention is not indicated. It is hoped that this may help the service user to avoid a hospital admission or further deterioration to their mental health and well-being.

    The duty worker would be expected to have excellent communication, time management and prioritisation skills - all of which are essential to ensure that concerns are appropriately considered, escalated and any outcomes actioned in a timely manner. We would expect the duty worker to have extensive experience in risk assessment and management.

    The Team is guided by collective decision making, providing evidence-based psychological and medical interventions to patients with often complex mental health difficulties. The patients journey is guided by the biopsychosocial formulation of their presenting difficulties, which subsequently inform the development of a collaborative plan of care for the patient. We draw from evidence-based interventions to provide high quality and effective interventions along side appropriate safety plans. The Duty Worker will be able to use these plans of care and associated risk management documents to guide their interactions and interventions.

    The applicant will need to have up to date knowledge on mental health service delivery frameworks including but not limited to, safeguarding of adults and children and appropriate methods of escalation of any concerns.

    Your role will involve working closely with other partners, agencies, carers, service users, families and the local community, ensuring continuity of service is provided at a high level.

    Knowledge

    Essential

  • Ability to demonstrate appropriate evidence-based knowledge around practice of caring for patients with poor mental health and complex conditions.
  • Understanding of relevant legislation (. Mental Health Act, Mental Capacity Act).
  • Clinical Risk Assessment and Management and its application in practice.
  • Understanding of the multi-disciplinary team and its role in shared decision making and how to use the MDT appropriately.
  • Qualifications

    Essential

  • Current professional registration with an approved professional body.
  • Evidence of Continuing Professional Development
  • For registered nurses: MIP, FLIP, ENB 998 Teaching & Assessing in Clinical Practice or City & Guilds 730 Teaching in Adult Education. Must be achieved within agreed timescale.
  • Recognised sign-off mentor, clinical educator or equivalent. Must be achieved within agreed timescale.
  • Key skills in literacy, numeracy and ITQ level 2 (or equivalent)

  • Working collaboratively with service users and their families/carers.
  • Significant experience working with people with mental ill health in a community environment.
  • Experience of completing risk assessment around complex mental health needs and conditions.
  • Desirable

  • Providing clinical supervision to individuals or groups as professionally appropriate.
  • Supervision of students on practice placement.
