Support Coordinator


We're looking for an enthusiastic and motivated individual to join the Waltham Forest Team in London, reporting to the Service Delivery Coach.


Position:
S1080 Stroke Support Coordinator


Location:
Homebased, Waltham Forest, London, however, regular travel will be required as part of this role (will include team meetings or other work related meetings)

Hours:
Part-time, 21 hours per week


Salary:
Circa £15,352 annum (circa FTE £25,500 per annum) (inner London weighting £1,979 per annum or outer London weighting £1,053 per annum may be applied in accordance to where you live)

Contract:
This is a fixed-term maternity contract until November 2024. Services are contracted and there is currently funding for this contract until 31 March 2024.

The Role


Key responsibilities will include:
*Organise and facilitate effective service communication support group activity for the benefit of stroke survivors and carers in understanding and achieving their goals.
*Ensure that timely, confidential and accurate records are kept on the CRM database and all data is in line with the retention policy and GDPR compliant.
*Develop and manage the service volunteer to support service delivery for stroke survivors and carers as required.

About You


You will have experience in:
*Organising and facilitating effective support group activities.
*Providing person centred support to empower vulnerable people or people with a disability or long term health condition and their carers
*Ability to nurture emotional resilience needed to handle a variety of calls, potentially dealing with complex and challenging situations whilst working in your own home


To fulfil the role, you must be a resident of the UK and have the right to work in the UK.
