    Clinical Lead/ Clinical Specialist Respiratory Physiotherapist

    We have an opportunity at the Whittington Hospital for an enthusiastic and forward-thinking clinician to join us as our Clinical Lead Respiratory Physiotherapist. The ideal candidate will have advanced knowledge and proven skills across acute and critical care services in the care and treatment of complex respiratory patients, including trauma patients.

    The post holder will provide clinical, professional and strategic leadership for respiratory and critical care physiotherapy across the organisation working closely with Consultants in respiratory medicine. The NIV service at the Whittington Hospital is led by Physiotherapy.

    The post holder will also manage and have clinical responsibility for the complex respiratory and bronchiectasis outpatient physiotherapy service.

    This role will have dual clinical and managerial responsibilities and would be ideal for someone wishing to keep a clinical focus whilst developing or progressing skills in operational management and leadership.

    The successful candidate should be able to demonstrate significant clinical, research and education experience and ideally some involvement in quality improvement work.

    Main duties of the job

    To provide clinical and managerial leadership to acute therapy services at the Whittington Hospital.

    To be responsible for the operational management of allocated teams supporting both clinically and managerially.

    To contribute to the strategic development of the Therapy Service including leading on any designated projects, service changes or developments.

    Flexible approach to working requiring the post holder to undertake on-call, late shift & weekend commitments as required.

    To provide clinical, professional, and strategic leadership for Respiratory and Critical Care Physiotherapy across Whittington Health NHS Trust.

    Be responsible for the effective provision, delivery & development of the Respiratory Physiotherapy service across the Trust.

    To demonstrate advanced clinical skills & provide specialist support & advice to Physiotherapy and Critical Care colleagues & others as required

    To hold a clinical caseload of complex respiratory & critical care patients as an autonomous practitioner.

    To have overall clinical responsibility for the 24hour emergency out of hours physiotherapy services (including on call & late shift services).

    To co-ordinate the 24hour physiotherapy led non-invasive ventilation (NIV) service for the hospital.

    Provide support & expert advice during working hours, evenings & weekends to physiotherapy staff working as part of the late shift and on call rotas.

    To deliver and manage the complex respiratory and bronchiectasis outpatient physiotherapy service.

    Detailed job description and main responsibilities

    MAIN DUTIES

    Clinical Lead Physiotherapist

    To work with the Clinical Manager for OT/PT in providing operational leadership to the In-Patient Therapy Services and as required to work with Director of Operations for Emergency and Integrated Medicine, Deputy Director of AHP's and other Service Managers in Whittington Health, in identifying opportunities for patient focused clinical service developments in line with Trust objectives and national policies.

    Professionally and clinically lead, motivate and supervise designated teams to ensure that safe effective patient care is delivered.

    To provide professional and managerial support to designated Team Lead Therapists (PT & OT) whose roles will include planning and coordinating the therapy required for designated speciality areas.

    Support Team leaders in delivering and evaluating the Therapy Service provided through quality assurance projects and audit.

    To coordinate and plan assigned rotas in conjunction with the other Clinical Leads ensuring that staff allocated have the appropriate skills and training to carry out late shift, weekend and on-call duties.

    To take part in the late shift, on call and weekend rota for Respiratory Care speciality, including working with and inducting new staff to the rota, and providing specialist telephone advice to less experienced staff out of hours. See Appendix 1.

    To maintain community and cross–hospital links with health and social care teams.

    Respiratory Specialist Role

    To provide an advanced specialist knowledge and physiotherapy skills to patients with a wide range of respiratory conditions, across acute specialities, including paediatric patients and to A&E on an as required basis; and to maintain associated records as an autonomous practitioner.

    To carry out expert assessment and management of patients as an autonomous diagnostic practitioner.

    To be a source of highly specialist clinical knowledge and an expert in the assessment, treatment and management for all types of respiratory conditions seen in an inpatient setting. All underpinned by expert clinical reasoning skills.

    To be the lead Therapist for the ward-based NIV service, from initial set-up to monitoring and weaning patients throughout their admission and providing specialist training on NIV/NIV set up to others. Additional responsibilities include teaching relevant staff groups, supporting on call staff out of hours and maintaining communication with the medical team, in particular the Respiratory Physicians.

    To be responsible for the advanced assessment, management and treatment of patients within the speciality who have more complex/chronic presentations, to determine the appropriate treatment indicated and advise, when necessary, other professionals, including medical staff, about the best course of intervention for that patient.

    To be responsible for planning and management of the physiotherapy late shift/on-call service, ensuring that policies and procedures are in place, all rosters are covered, and that training and induction is provided for all physiotherapists on the 10:30 to 18:30 late shift rotas.

    To provide support and expert advice via telephone to on call physiotherapists out of hours.

    To take part in the on call and weekend rota for Respiratory Care speciality, including inducting new staff to the rota. See Appendix 1.

    To undertake clinical audit as well as education and training of Allied Health Professionals, Nursing and Medical Colleagues as a major component of both roles.

    To be a specialist resource and active contributor to trust wide education and training programmes for Medical, Nursing and Allied Health Professionals (AHP) in fields of acute respiratory care and rehabilitation.

    To teach and impart knowledge on acute and chronic respiratory conditions and management of these to Band 7 – 3 Therapy staff, students and staff from other disciplines.

    To advise on the strategic development of respiratory inpatient and critical care services at the Whittington

    To identify clinical audit, research and data collection priorities to drive clinical development for both services.

    To act as a resource and advisor to colleagues from other disciplines at the Trust regarding the appropriateness of respiratory physiotherapy input.

    Through updating of protocols and evidence-based projects to make recommendations for changes in practice or to improve service delivery and use tools such as audit and outcome measures to evaluate the effectiveness of own and team's practice.

    To work alongside the Respiratory Physicians in developing the NIV and respiratory services within the hospital and local community.

  • Skills and Abilities

    Essential criteria

  • High Standard of written and spoken English. IELTS Level 7 for international candidates
  • High level interpersonal skills such as negotiation, counselling and communication including where barriers to communication are present.
  • Ability to relay and interpret complex, sensitive or contentious information e.g., to individual and groups of patients, staff and multidisciplinary teams.
  • Ability to develop, implement and evaluate individual programmes of care for respiratory patients with multiple pathologies.
  • Proven clinical leadership skills.
  • Specialist skills in acute respiratory care.
  • Advanced teaching and presentations skills to patients, carers, students, all levels of physiotherapy, OT staff and other professionals using a variety of formats including specialist training.
  • Evidence of excellent clinical reasoning skills.
  • To be able to develop and implement service development policies and standards for Acute Respiratory Disease
  • Excellent organisational and time management skills.
  • Evidence of team building skills.
  • Ability to manage human resources procedures such as staff appraisal, performance management, recruitment and interviewing.

    Essential criteria

  • Minimum 5 years post-graduate experience with minimum 1 year at band 7 level in respiratory setting
  • 4 years in relevant areas e.g., acute and community, adult and paediatric respiratory care
  • Previous experience as a Specialist Band 7 or Team Leader
  • Wide range of Senior experience in providing Respiratory Physiotherapy
  • Minimum of 1 years prior experience in the NHS or similar organisation
  • Previous experience of working in multidisciplinary teams as an active participant.
  • Current or recent experience of on call and weekend work
  • Knowledge and experience of supervision and appraisal – self and others.
  • Postgraduate teaching to all levels of physiotherapy staff.
  • Demonstrate awareness of current developments and best practice in acute respiratory areas and acute clinical specialities.
  • Demonstrate an understanding of setting and maintaining standards of care for respiratory patients
  • Demonstrate specialist knowledge of the assessment, management and rehabilitation in critical care settings
  • Detailed knowledge of relevant National and Professional Guidelines
  • Good working knowledge of health and social care pathways for the hospital patients
  • Excellent technical knowledge of rehabilitation and enablement equipment essential for the delivery of care for respiratory and complex patients
  • Evidence of CPD and ability to manage own learning in the form of objectives, reflective practice and career development plans
  • To have advanced computer literacy skills in the form of word, excel, graphs/charts, presentations and documentation templates
