Gellinudd Recovery Centre is a Locked Rehabilitation Mental Health Hospital based in the Swansea Valley.
The hospital provides individually recovery-focused care to female patients with complex needs in line with Adferiad Recovery's values as Wales's leading co-occurring mental health and substance misuse charity.
The vacancy is for a full time Recovery Practitioner with suitable experience in working with service users with mental health difficulties.
You will be expected to act as a support worker within the hospital, supporting the staff nurses in their assessment, planning, implementation and evaluation of care.
Duties will include supporting individuals with their needs both at the hospital and within the community by developing a therapeutic relationship to help people achieve their goals in recovery.
You will be joining a team of recovery-focused staff comprising Consultant Psychiatrists, a Consultant Psychotherapist, Occupational Therapist, Mental Health Nurses, Recovery Practitioners and Peer Support Workers.
Responsibilities:
- Work with patients carers and staff in a recoveryfocused manner, demonstrating commitment to a patients journey through the mental health system.
- Work closely with patients to meet the needs and achieve the goals identified in their individual care plans.
- Provide practical support to patients and their carers to meet activities of daily living. Being mindful to promote skilldevelopment and independence as far as possible.
- Escort patients into the community on preauthorised leave, ensuring that the patients have their needs met safely with the clear knowledge of the Nurse in Charge.
- Maintain accurate and timely records of all contact with patients/ carers/relatives and other clinical contacts.
- Ensure all incidents which may compromise Health and Safety are reported in an appropriate and timely manner.
- Have knowledge and understanding of patients risks, their risk assessments, risk factors, reporting the patients condition to a registered nurse to ensure the delivery of safe care.
- Work with a range of other professionals in a supportive and professional manner, including peer support workers with lived experience.
- Provide written and verbal reports based on facts and observations.
- Must be able to undertake Formal Restrictive Physical Intervention Training with annual updates.
- Observe the safety of patients at all times and if requested account for their whereabouts.
- Assist with safe and supportive levels of engagement of patients deemed to be at risk of self harm.
Person Specification:
- Significant experience of working in mental health care.
- A good level of understanding of how mental illness can impact on motivation, social skills and activities of daily living.
- Full driving licence and willingness to utilise company vehicles to transport patients.
