Job overview:


We are seeking to recruit a dynamic, enthusiastic and experienced therapist to support in the two hour therapy response across Cheshire West.

As pressure on emergency departments across the UK grows, new care models will be pivotal in delivering and relieving some of these pressures by diverting work away from the hospital and into the community.

The NHS England paper 'Community health services two hour crisis response guidance' aims to address this pressure.


Main duties of the job:


We are looking for a highly motivated, well organised and flexible Physiotherapist or Occupational Therapist to provide assessment and treatment as part of a collaborative approach across the Integrated Care Partnership.


The successful post holder will work closely with our Hospital at Home Service and Care Community Teams to support patients to remain at home during episodes of acute health deterioration and reduced functional ability, with the aim of preventing unnecessary attendance at A&E, providing a therapy response within 2 hours.

The role is a great opportunity to make a difference to patient outcomes using knowledge and skill from all areas of clinical practice.


You will need to be able to work autonomously and have excellent time management and prioritisation skills and be keen to work in a collaborative way with services and patients with sometimes complex presentations.

The positions are to cover the service between the hours of 8am - 8pm 7 days per week.
