To provide Support and Time to an allocated group of service users in order to promote their recovery and physical health monitoring to maintain them in their community environment helping them to build their own support network.

To support the individual service user to identify their needs and have these at the fore at all times, working to the Recovery Model to support service users to work towards their identified goals.

To support the individual service user with their physical health needs To support the on-going development of the physical health clinics within community mental health services.

To support the clinical team in the delivery of high quality health care to the patients To support the running of Clozapine Clinics.
