- GCSE level C or equivalent for literacy and numeracy
- EITHER completed or working towards completion of a relevant Foundation Science degree or equivalent OR NVQ level 3 or equivalent and experience/skills demonstrated through CPD to include clinical reasoning, appropriate attitudes and behaviours, initiative and leadership.

Job summary
An exciting opportunity has arisen for enthusiastic individuals to join our Frailty Assessment team as band 4 Senior Support Worker(s) on a substantive contract. The role involves working alongside senior frailty therapists and the frailty multidisciplinary team to provide a rapid assessment of older adults, initiating services where required to safely avoid unnecessary hospitalisation and deconditioning. Contributing towards completing a comprehensive geriatric assessment to those individuals attending the Emergency Department (s) Frailty Assessment Unit and Frailty Direct Attend to achieve a positive outcome for the patients.
Main duties of the job
The post holder is part of the Frailty Multidisciplinary team providing a rapid assessment of older adults, initiating services where required to safely avoid unnecessary hospitalisation and deconditioning.
Contribute to service delivery by independently managing a caseload of delegated clinical and non-clinical tasks under the guidance of a qualified therapist.
To work as a proactive member of the Frailty team, contributing to comprehensive geriatric assessments by assessing, implementing and monitoring treatment plans of patients, performing specific clinical duties within levels of competency deemed appropriate for this post.
Demonstrate an excellent awareness of community pathways and referral processes to enable timely, safe discharges from the acute trust.
Support the learning and development of new starters and students.
Develop and maintain communication with a wide range of patients, carers, health care professionals and other departments as appropriate.
Perform administrative duties as required supporting the Frailty Therapy team and wider Frailty team.
Ensure equipment is available for treatment and responsible for checking it is safe for use prior to issue.
