Job summary
We are looking to recruit a suitably qualified Care Coordinator to work in our expanding multidisciplinary team (MDT) on behalf of Bridlington Primary Care Network member practices.
They are seen as pivotal to our planned work and are evolving roles in support of staff and practices in delivery of effective care and services.

Overview of your organisation
Welcome to Bridlington Primary Care Network
From 1 July 2019, all the practices within East Riding of Yorkshire came together and developed seven PCNs.
Each network has appointed to the role of Clinical Director (two networks have this as a shared role between two people).
The networks are in their early stages of development; they have begun to meet regularly as a team and will go onto engage with wider health and social care providers (including the CCG) and patient groups to discuss how they collectively plan to improve services for their population.
They pull together existing information in one place and contain data on a range of indicators for local populations, highlighting issues that can affect health in each locality.
The profiles are intended to help local government and health services make plans to improve the health of their local population and reduce health inequalities.
YHP is a GP led organisation established in 2013. It has 15 member practices with a combined list size in excess of patients.
It was initially established to support independent partner led general practice by holding contracts for primary care as well as focusing on the principle of care closer to home.
YHP seek to provide a strong voice for primary care, patients, and staff, both as a separate entity, and in partnership with others.
Job Summary
Working as part of the Primary Care Network (PCN) multidisciplinary team alongside Social Prescribing Link Workers and Health and Wellbeing Coach(es) you will support the provision of an all-encompassing approach to personalised care and promote this being embedded across the PCN.
Key Areas of Responsibility
- Proactively identify and work with cohorts of patients to deliver personalised care.
- Support patients to utilise decision making tools to prepare for shared decisionmaking conversations.
- Working holistically, bring together their identified care and support needs and explore options to meet these within a single Personalised Care and Support Plan (PCSP).
- Help people to manage their needs by answering their queries, making and managing appointments, providing good quality written or verbal information to inform decision making and enhancing their knowledge, skills and confidence.
- Support people to take up employment and training and to access appropriate benefits through referral to the Social Prescribing Link Workers.
- Connect patients with tools to support them, such as selfmanagement education, peer support and the adoption of healthy behaviours.
- Provide coordination and navigation for people and their carers across health and care services, working closely with the Social Prescribing Link Worker, Health and Wellbeing Coaches and other health professionals.
- Support the coordination of MDTs across the PCN.
- Raise awareness of the benefits of shared decision making and decision support tools, how to use these and how to identify patients who may benefit from this approach.
- Ensure all VCSE organisations have appropriate safeguarding procedures in place and are genuine opportunities for patients to develop a sense of belonging, build knowledge, skills, and confidence.
- Discuss concerns with a relevant GP to ensure safeguarding procedures are followed appropriately.
Act on own initiative in prioritising and managing workload, in respect of clinical, managerial, and administrative areas, as necessary.
Maintain own personal and professional development as agreed at your annual review.
