    Job summary

    Social prescribing empowers people to take control of their health andwellbeing through referral to non-medical social prescribers who give time tofocus on what matters to me. This role specifically works with individualswith chronic lung conditions such as COPD and Asthma, by taking a holisticapproach to their health and wellbeing, by creating personalised care andsupport plans and coordinating appropriate support and interventions.

    Main duties of the job

    The successfulcandidate will play a key role in proactively identifying and working withpeople, including the frail/elderly and those with long-term conditions, toprovide coordination and navigation of care and support across health and careservices.

    They will workclosely with GPs and multidisciplinary teams, making sure that appropriatesupport is made available to people; supporting them to understand and managetheir condition and ensuring their changing needs are addressed. They willenable people to access the services and support they require to meet theirhealth and wellbeing needs, helping to improve peoples quality of life andlive independently.

    They will workalongside a multidisciplinary team to provide an all-encompassing approach topersonalised care and enable people to navigate through the health and caresystem. The work will be led by the wants and needs of the individualsthemselves and may need some flexibility in working hours to fit availabilityof these people.

    The post holderwill work with a diverse range of people from different cultural and socialbackgrounds. The ability to work confidently and effectively in a varied, andsometimes challenging environment is essential.

    The successfulcandidate will have excellent interpersonal and communication skills, and beorganised, patient and empathetic. They will have experience of working inhealth, social care or other support roles including direct contact withpeople, families, or carers.

    Job description

    Job responsibilities

    Toact as a main point of contact for individuals accessing the service.

    Accept referrals and assess how far a patientshealth and wellbeing needs can be met by services and other opportunities inthe community.

    Manage and prioritise your caseload reflecting yourpatients needs.

    Together produce a simple personalised care andsupport plan to address their health and wellbeing needs.

    Develop trusting relationships by giving people timeand focus on what matters to them.

    Following a patients journey, evaluate how far theactions in the care and support plan are meeting their health and wellbeingneeds.

    Proactivelyidentify and work with people, including the frail/elderly and those withlong-term conditions, to provide coordination and navigation of care andsupport across health and care services.

    Where required refer patients back to other healthprofessionals within the team.

    Liaisewith appropriate statutory and voluntary sector agencies working in Public& Mental Health, Primary and Secondary Care, Social Care and build upappropriate networks that promote timely access to support individuals.

    Towork systemically, liaise with and include families/carers, healthprofessionals etc. when appropriate and as agreed by the service users.

    Tobuild strong working relationships with internal and external services and topromote the link worker/social prescribing model.

    Work collaboratively with local Voluntary, Communityand Social Enterprise (VCSE) organisations and community groups.

    Ensure data is captured in the clinical systems andSNOMED codes are utilised.

    Discuss concerns with a relevant GP to ensuresafeguarding procedures are followed appropriately.

    Toattend multi-disciplinary team meetings (MDTs) as required.

    Toensure individuals are made aware of key processes complaints, informationsharing, getting involved, and to make sure that they are asked about how theywould prefer information.

    For your own personal and professional development,you will be expected to participate in monthly supervision with a healthprofessional.

    Toattend mandatory training and abide by policies and procedures on safeguarding,health and safety and equality and diversity.

    Toattend appropriate continuing professional development and training events,team meeting and supervision sessions.

    Tocarry out, from time to time, any other appropriate duties in line withcapabilities as directed by the line manager.

    Managementof workload and personal development

    Act on own initiative in prioritising and managingworkload, in respect of clinical, managerial, and administrative areas, asnecessary.

    Maintain own personal and professional development asagreed at your annual review.

  • Good standard of secondary education including Maths and English
  • Has undertaken the NHS England and NHS Improvement online learning programme
  • Enrolled/in progress/qualified from appropriate training as set out by the Personalised Care Institute
  • Desirable

  • Has safeguarding knowledge either level 2 or the equivalent experience
  • Training in motivational coaching and interviewing or equivalent experience
