    Job summary

    Exciting opportunity to join our established Home 1st Twilight Nursing team in Milton Keynes. We are recruiting staff to work alongside our qualified nursing team, delivering out of hours, unplanned care to patients in Milton Keynes.

    Twilight operational hours provide flexibility for care responsibilities outside of work.

    In this role you will ensure the most appropriate care is delivered to patients in their own homes, whilst working alongside registered nurses.

    Full driving licence and access to a car for work is essential to this role.

    Home1st is based in Bletchley and the modern office is a central hub for multi-disciplinary working. You will be provided with uniform and have mileage expenses paid for.

    The successful applicant may have contact with patients or service users.     Main duties of the job

    To work as a Health Care Assistant to support the multidisciplinary team, in the effective and efficient provision of care to aid patient's recovery, promote independence, health and wellbeing following sudden illness or deterioration.

    Provide support to registered nurses, working as a team providing individualised care plans as set by nurses.

    Work as a member of the multidisciplinary team observing, recording and reporting any changes in a patient's condition or circumstances to registered staff.

    Be familiar with the steps necessary in an emergency and be able to act accordingly.

    To provide planned, delegated nursing interventions for patients in their own homes or other settings as appropriate.

    To participate in shared care with other agencies / services as required.

    Provide accurate feedback on patient's care to Nursing teams.

    Ensure the needs of carers and relatives are listened to and reported back to the Nurse in Charge of the shift.

    Ensure clear, concise and accurate records are kept and updated in a timely manner, including the use of departmental electronic records.

    To be aware of health and safety and follow risk assessments to ensure the safety of service users and staff.

    To undertake relevant training as provided.

    Apply knowledge/skills gained from education/training so that a high standard of care is maintained at all times.

    Job responsibilities

    Assist with personal care as required, promoting independence and ensuring dignity and respect in care.

    Carry out observations of vital signs according to competency and under instruction from registered nurse.

    Observe, record and report to Nursing Staff any change in patients condition or circumstances.

    Undertake delegated duties according to patients individualised care plan.

    To carry out pressure area care, eye care, catheter care, stoma care, personal care and collection of specimens / medicines and equipment as per care plan and in line with agreed competency .

    To obtain consent to any procedure from each patient and to proceed only when consent is obtained. Document when procedures are carried out in patients best interests.

    Be familiar with the steps to be taken in medical emergencies and act accordingly.

    To maintain accurate records of intervention on the patients electronic records on each shift and in line with the CNWL care record standard policy.

    To use equipment appropriately as described in the patients care plan.

    To attend in-service training, clinical supervision and team meetings as required.

    To work across Home 1st Services when there is a need.

    Person Specification

    Qualifications

    Essential

  • oGood level of general education (GCSE or equivalent)
  • oCare Certificate or the willingness and ability to undertake
  • Knowledge & Experience

  • oPrevious experience of working as a Health Care Assistant.
  • oKnowledge and understanding of the importance of person centred care
  • oKnowledge and understanding of the importance of independence and care closer to home.
  • oAbility to question, clarify, knowledge of own limitations and importance of risk assessment
  • oAwareness about confidentiality
  • Desirable

  • oPrevious experience of working directly with older patients is desirable
  • oNHS experience is desirable
