Communication and Relationships Skills To provide and receive complex, sensitive and challenging clinical information e.g.

condition related communication with patients and relatives, where there are barriers to understanding To communicate information to effect, change in practice and ensuring the effective dissemination of new knowledge To communicate complex information in partnership with the patient and relatives.

This will involve using inter-personal skills where agreement or co-operation, reassurance, empathy is required e.g.

clinical assessment Professional networking locally and regionally Analytical and Judgemental Skills To assess the needs of patients and their relatives based on the analysis of a range of facts or situations.

Interventions will be planned on the assessment information and the interpretation of this Obtains, analyses and interprets assessment information, presenting symptoms and physical health to develop the appropriate interventions Demonstrates critical thinking and diagnostic reasoning skills in clinical decision making Planning and Organisational Responsibilities To prioritise, plan and manage a defined caseload, delegating tasks to others as appropriate Responsible for managing and prioritising own workload, balancing the needs of patients, their families and service requirements/competing priorities Physical Skills Standard keyboard skills Advanced dexterity and or sensory skills Responsibilities for Patient Care Provides advice, assessment and interventions/programmes of care for patients and other professionals as appropriate Develops and maintains the patients control over decision making, assess the patients commitment to the jointly determined plan of care, and fosters personal responsibility for health and well-being Deliver care interventions and advice in a variety of settings that promote patient flexibility/choice, and which are sensitive to cultural preferences Responsibilities for Policy and Service Development Implementation Follows policies, protocols, standards in own role may contribute to reviewing, developing and monitoring operational/clinical policy, within the clinical areas Responsibilities for Financial and Physical Resources Responsible for the safe use of equipment used in course of treatment and equipment used by patients and others Responsibilities for Human Resources Provide clinical supervision and or training within own area of work to other professionals, patients, relatives Be on the live register of mentors following preceptorship Responsibilities for Information Resources To provide accurate and timely information to inform Trust/National IT systems as required Maintain up to date training and basic IT skills, and demonstrate a good working knowledge of confidentiality and data protection Responsibilities for Research and Development To participate/undertake audits/surveys in own area of practice Freedom to Act To work within professional code of conduct, organisational policy and guidance To work as an autonomous practitioner and is accountable for ones own action working within the professional competency framework Work is managed rather than supervised and remains accountable for all delegated work to other members of the team
