The post holder will support the Clinical Triage staff within the Nottinghamshire ICB Transfer of Care HUB.

The role will be pivotal in organising and supporting discharges from bed based services across Nottinghamshire and transitioning into community healthcare services provided by Nottinghamshire Healthcare.

These will primarily be across South Nottinghamshire County.


The post holder will be responsible for the effective coordination of patient discharges from bed based services to community services.

To optimise patient flow and appropriate place based care, including supporting admissions to step up beds as required.

They will work closely with clinicians, social workers, carers, third sector - Age UK and all multidisciplinary team members to deliver a high quality patient focused service, focusing on a Home first approach.

They will meet the required standard to proactively overcome identified concerns /issues that may delay effective discharge or admission into community services.

They will coordinate with clinicians within the Transfer of Care Hub and those in our community teams to co-ordinate assessments of patients' needs and initial reablement requirements and support the execution of the discharge plan.


The role requires the Discharge Co-Ordinator to have organisational skills to manage their own workload, and skills to support the Community teams team in managing the patients' journey out of bed based services and into the community.

This pathway will ensure care is delivered in the most appropriate environment by ensuring an effective and timely discharge. Furthermore, the role will organise and support the timely assessment within the community.

The individual will need to be highly motivated and have effective interpersonal skills; able to maintain the delivery of a high quality service during a process of change.

The post holder will develop innovative new ways of working with other providers to enhance improved working practices and relationship across health and social care and contribute to the delivery of the Discharge 2 Assess agenda.

The post holder will possess skills that enable them to accurately refer on to other third party services as the patient requires.

The post holder will have effective communication skills which will be utilised with a wide MDT and hospital based teams to ensure that all relevant information is accurately shared and communicated across parties.

The role will be based in the Transfer of Care Hub, which is currently at Nottingham University Hospital. The service runs over 7 days so some weekend work will be required - on a rota basis.

We support the discharge of over 400 patient per month and manage the future health needs of these patients within a community setting.

The post holder should have good IT skills, a flexible approach, able to challenge appropriately.
