JOB DETAILS

TITLE:2x Care Co-Ordinator - 25 hours per week


HOURS OF WORK:
Contractually 8.00am to 8.00pm Monday to Sunday,
These position are currently on rota for the hours of
8am to 3pm or 12pm to 6.00pm


SALARY:
Level 1 - £21,450 - £22,425 pro-rata
£11 - £11.50 per hour DOE

ABOUT US
The current list size of Danetre Medical Practice is approximately 12,800.

Any increase over the last ten years has been due mainly to; natural change (more births than deaths); building development within Daventry itself and more recently migration from the rest of the UK and overseas.

The boundaries of the Practice show we are large in terms of geographical size, with the majority of the land classified as rural.

The practice itself is located in close proximity to Daventry Town centre in a modern building adjoining Danetre Hospital (a community hospital which opened in September
2006) offering a wide range of services and delivering a high standard of care. Also attached to the Practice operating independently is Boots Chemist.

Currently the practice works with 5 x GP partners, 1 x Managing Partner, 4 x salaried
GP's, a comprehensive MD Team and over 250Hrs of Clinical nursing support.


ABOUT US ADDITIONAL INFORMATION
The post holder holds a position of importance, being the crucial link between the doctors, staff and patients.

Full training will be given in all aspects of the job.

Telephone calls are monitored and used for training purposes and to accurately record patient and GP information.

You will be required to wear a uniform blouse and a name badge, which will be provided for you.

You will be expected to attend regular staff meetings. The meetings provide an opportunity to discuss matters arising from the job and can be used for in-house training.


JOB PURPOSE


To work as part of a team delivering a first class, professional patient service, that helps to manage a patient's care, for example, the elderly, disabled or at risk patients using a range of communication and administrative methods.


RESPONSIBILITIES

KEY
- help patients to manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care
- provide coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers and other primary care professionals
- providing a comprehensive link with local Care Homes to manage and monitor medicines and the registering of new patients, working closely with the Clinical

Pharmacists and Prescribing Technician
- work as part of a Multi-Disciplinary Team, in Practice, managing the Ageing Well programme
- following up on both acute and planned discharges from Secondary Care
- assisting clinical and Management team with ensuring all targeted areas within the

Quality Outcomes Framework/IIF/NHSE/PCN DE, are met
- facilitating ad hoc targeted clinics as and when required, providing exceptional service to sometimes elderly and/or vulnerable patients
- providing support to vulnerable and elderly patients with medicines management
- manage patient waiting lists


GENERAL

  • To work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures whilst managing own workload to deliver the practice priorities.
  • Adhere to established procedures and agreed Key Performance Indicators to enable quality standards to maximise patient satisfaction
  • Support the development and implementation of technology and systems
  • Using DMP's systems and databases enter, store and/or retrieve, summarise information as requested or otherwise necessary
  • Ensure that every consideration is given to patients and visitors at all times.
