The purpose of the role is to provide a live and same day remote screening service for patients experiencing mental health crisis (and other mental health symptoms), their families, carers; and other Healthcare Professionals including LAS, and the police.

The postholder will keep service users as safe as possible, through use of sound clinical skills and effective risk assessments, signposting or referring patient to appropriate services.

This is a fast paced and demanding role, but is also incredibly rewarding.

This role will be hosted by South London and Maudsley, however will work in partnership with South West London and St Georges NHS Trust and Oxleas NHS Trust.

Clinical

  • Triage all calls coming through to the Hub
  • Signposting callers to other Trust services where appropriate
  • Referring patients and carers to appropriate downstream MH and support services
  • Seek advice and support with regard to escalating concerns about an individual user to the Crisis hub Practitioner:
  • Seek and document advice from senior MHCHP in order to continue the call
  • Be clear at which point to handoff the call to a B6 MHP in order continue the call based on details already gathered
  • To ensure that all telephone enquiries to teams are handled politely and sympathetically, ensuring that all onward referrals are completed in a timely and accurate manner which may often involve dealing with difficult and distressed patients on the phone
  • To maintain the need for confidentiality of service users and professional information
  • Ensuring all calls are managed in line with the Standard Operating Process (SOP) for the service
Professional

  • To ensure that documentation of care of individuals is maintained to NMC standards.
  • To carry out risk assessments and to seek support and advice from a senior clinician where necessary.
  • To be trained in customer care and brief interventions and be able to direct patient calls to the Band 6 Clinical Advisor or, Emergency Services

The Assistant Practitioner will demonstrate robust assessment, risk assessment and management skills and possess sound decision-making skills for people in a mental health crisis, managing professional boundaries and knowing when and if to escalate to enable formulation of complex triage forward plans under the guidance of registered clinicians.

The role requires to be able to work flexibly around a 24/7 shift pattern as part of a team.
