An exciting opportunity has arisen for us to be able to recruit more Rehab Support Workers to join and enhance our community rehab team.


You will join the Integrated Care Service (iCares) which works across Sandwell providing rehabilitation, case management and admission avoidance to people in their own homes.

These are often people recovering from an illness, a hospital stay or increasing frailty.

With our support and specialist rehabilitation we aim to improve their physical function such as walking, looking after themselves and even returning to community activities again.


Within the Team we have Occupational Therapists, Physiotherapists, Community Matrons, Speech and Language Therapists, District Nurses, and Assistant Practitioners all working together to deliver integrated care.

  • To work as a member of the service providing a rehabilitation approach to clients.
  • To participate in the assessment, treatment and monitoring of clients referred to the service.
  • Under supervision, to manage a caseload of clients to meet patient and service demands.
  • To provide interventions to clients as agreed with qualified staff, working collaboratively to meet ongoing client needs.

Within iCares we operate a 7 day service 8am-8pm, we take it in turns to contribute to weekend working and 8pm finishes.
