Description

An exciting opportunity has arisen to work as a 'Dementia Link Worker' in a collaborative role through a working partnership between Community Links, Primary Care Network - Five Parks and Working to Wellbeing Community Partnership.

As a Dementia Link Worker, your mission will be to transform the way we deliver healthcare, especially for those navigating the complexities of dementia.

**If you have

  • A creative, flexible and imaginative approach to working with people from vulnerable groups.
  • Experience working with people with dementia either as a carer, worker, or volunteer.
  • The ability to undertake work with high degree of accuracy and strong attention to detail.
  • Working knowledge of dementia and brain health to provide Information and Guidance.
  • Access to a car for work purposes and hold a full current UK driving licence.
