- oExcellent knowledge & understanding of recovery principles and NICE guidance in relation to common mental disorders and the ability to ensure that evidence based practice with clear clinical outcomes is embedded into practice.
- oDemonstrate a knowledge base for substance use and associated risks.
- oSelf-aware demonstrating a positive attitude on achieving the best results and overcoming obstacles.
- oEssential car driver, must have access to transport to drive between bases
- oAbility to remain calm under pressure, motivate others and be flexible and adaptable to change.
- oAbility to communicate positive expectations of the organisations (PCN and ELFT) to others in an enabling and supportive manner.
- oAbility to work independently and as part of a team and able to both/influence and work across professional boundaries.

Main duties of the job
Work both with individuals that are presenting with a first presentation of mental health need and those with on-going mental health needs that do not require support from the core mental health team, and to carry out annual reviews as needed.
To undertake physical health checks for those patients on the SMI register and facilitate follow up support as required.
Work as an autonomous practitioner to deliver a range of mental health interventions with support from the senior mental health practitioner and the wider allied core mental health team as required.
