Remedy Recruitment Group are currently seeking a Senior Social Worker for the Adult's Mental Health Team.You will need to be:
- SWE registered
- Eligible to work in the UK
- Carry overall responsibility for the management of a caseload, acting as a care coordinator, as defined by the Care Programme Approach (CPA).
- Deliver effective care and treatment that is carried out within a multidisciplinary framework and in collaboration with other agencies.
- Involve service users and their carers in planning care that includes all of their individual and diverse needs.
- Design individual programmes of care collaboratively with patients to maximize self management and social inclusion and reflect their own goals and aspirations. Review and adjust plans according to changing service user needs.
- Provide evidence based therapeutic interventions using developed interpersonal skills such as CBT or psychoeducation.
- Provide service users and their carers with clear and intelligible information which will assist them in making informed decisions about their treatment and care.
- Administer medication, monitor for effects and sideeffects, in accordance with NMC Standards and Trust policies.
- Facilitate vocational rehabilitation and those aspects of mental health recovery concerning occupation.
- Maintain clear, comprehensive and contemporaneous clinical records on all patients in response to personally generated clinical observations, in accordance with professional standards and Trust policies.
- Prepare and present clinical reports as required
- Provide formal, ongoing clinical supervision and caseload management supervision to junior and less experienced community mental health staff, as directed.
- Undertake teaching of qualified and unqualified staff, including students
- Undertake surveys or audits as necessary to own work and participate in clinical trials/R&D activity as required.
- Initiate and/or collaborate in service/clinical developments based on appropriate research evidence. Set, monitor and ensure maintenance of standards of care and clinical outcomes.
- Promote health and provide specialist advice/education regarding mental heath to professionals, service users and the public in general.
