Description

An exciting opportunity has arisen for a part time Band 4 Resettlement Support Worker to join the Later Life Service supporting vulnerable Older Adults with housing issues.

The Resettlement worker will work alongside community and ward based staff in supporting service users with mental health difficulties with any housing related issues and in setting up in new accommodation.

They will be working with people who are on the wards and whose discharge has been delayed due to lack of appropriate accommodation as well as working with community service users whose tenancies are at risk, or where their housing is having a negative impact on their health and wellbeing.

We are looking for a motivated and enthusiastic individual to join our friendly team.
