You'll work primarily on our 10 bedded Acute Frailty Unit (including a rapid access assessment bed for ambulance services) & in the Emergency Department, supporting the therapy staff in assessment and management of frail patients.


You'll have the opportunity to contribute to the Comprehensive Geriatric assessment & management, along with supporting those patients on the Post Discharge Frailty Support Service.

We're also exploring the expansion, Home to assess, to assist in early discharge - so as you can see, there's lots to do & get involved in.


We'll support you all the way with relevant training, giving you the chance to learn directly from our experts across many specialties which should stand you in good stead wherever your career goes next.


We know you'll be passionate in your care of frail patients, supporting the team in developing the service and in the education of other.


To work within the Integrated Frailty Team based on the Acute Frailty Unit (AFU) at Royal Preston Hospital as part of an expert clinical team delivering quality care within the specialised area and under mínimal supervision from a qualified therapist.

To work in close liaison with the MDT/ patients/ NOK / carers and community colleagues, to ensure effective discharge management and a seamless transition of care, to reduce length of stay and maximise efficient use of AFU beds.


Our fab team, known as LIFT, although small, is strong, really supportive & we manage to retain our sense of humour despite some of the pressures we face together and this is mirrored in the wider teams in which we work.

Although a relatively new unit - the acute Frailty ward has received extremely positive feedback.

Patients describe it as a warm, caring environment where staff make patients feel welcome and help put minds at ease when it comes to discharge.

To support personal and people development

To help to ensure the health, safety and security of users of the Acute Frailty Team, self and colleagues

To support service development and planning

To support the Clinical Governance and Quality agenda within Core Therapies

To support equality of opportunity and access within the Core Therapies for both staff and patients

To support financial and other resource management within the Core Therapies

To provide and use information to support admin processes and patient care

To directly support the delivery of therapy to patients
